What Is Cryolipolysis?

Cryolipolysis is a non-invasive fat-reduction technique that uses controlled cooling — applied through a vacuum applicator, as in CoolSculpting — to selectively damage fat cells while sparing skin, nerves, and muscle. The body clears the affected cells over the following weeks to months, producing a localized reduction in fat-layer thickness.

How it works

Fat cells are more sensitive to cold injury than surrounding tissue. A cooling applicator holds the treatment area at a temperature intended to trigger fat-cell death without harming the skin or deeper structures. Treated cells are gradually cleared by the body’s normal processes; results build over roughly 8–12 weeks per treatment.

How effective is it?

A systematic review of 19 studies found consistent fat-layer reductions of 14.67–28.5% by caliper measurement and 10.3–25.5% by ultrasound, across treatment areas including the abdomen, flanks, thighs, and arms. Blinded reviewers correctly distinguished before/after photos 79–89% of the time. Adding post-treatment massage improved results further (21% reduction versus standard protocol alone) (Ingargiola et al. 2015).

Is it safe?

Most side effects are mild and short-term: erythema, swelling, and pain that resolve within weeks. The same review found no measurable impact on lipid levels or liver function, no persistent scarring or infection, and no long-term nerve changes on biopsy (Ingargiola et al. 2015).

The notable exception is a rare but real risk: paradoxical adipose hyperplasia (PAH), where treated fat grows instead of shrinking. Current pooled data puts incidence around 0.22% (roughly 1 in 455) — considerably higher than the 1-in-20,000 figure often quoted by manufacturers — and correction requires surgery.
